Slim The Chin: Kybella Treatments Offer An Alternative Option To Surgery

Mirror, mirror on the wall, who’s the fairest of them all? Step aside, Snow White; men are paying closer attention to their reflections and investing in their looks with a newfound openness to cosmetic surgery.

Over the past 20 years, men seeking nips and tucks have shot up more than 325 percent, according to the American Society for Aesthetic Plastic Surgery. In order of popularity, men are lining up for nose jobs (rhinoplasty), eyelid surgery (blepharoplasty), breast reduction (gynecomastia), liposuction, and face-lifts, reports the American Society of Plastic Surgeons, and now account for more than 10 percent of plastic surgery patients.

While women have long felt the social implications of a world obsessed with youth, experts say that men are now feeling more of that pinch. “The reality is that the job market is fierce, and many of my patients come in to look and feel younger to boost their self-confidence,” says New York City plastic surgeon Douglas S. Steinbrech, MD, who specializes in male cosmetic surgery. “Guys want to stay competitive in the board room and feel and look good to clients and colleagues,” he says.

Is a chiseled jawline the new version of a shiny red midlife-crisis convertible? In addition to CEO and boardroom executive types, Steinbrech sees a lot of athletic men staving off the so-called “dad bod” with a “daddy-do-over” that tightens sagging faces and sucks away love handles and beer bellies that have slowly crept on over a couple of decades. Steinbrech even performs pectoral and butt implants on some male patients who want to recapture the jock look of college days gone by.

Whatever you may fancy, looking good doesn’t come cheap. Nose jobs, liposuction, and male breast reduction cost anywhere from $5K to $8K each on average, and face-lifts can run about $12,000. Eyelid surgery, which is approximately $5,000, is often an add-on to other anti-aging procedures for a completely refreshed look, since the eye area is often the first and most noticeable area to age.

Millennial men are also heading in to the cosmetic surgeon’s office, much earlier than generations before them, who often waited until retirement years to address advanced aging issues. They’re much more open and comfortable with the idea of plastic surgery, says Steinbrech. This age group tends to opt for more affordable injectable fillers and “Bro-tox,” which are often priced from only a couple hundred to a few hundred dollars and yet still significantly refresh the face.

Part of the draw is that cosmetic surgery — when performed by a board-certified and experienced doctor — is having far more natural results on men these days. Fears that you’ll end up looking like an aging D-list celebrity in a tabloid headline are diminishing. This month, Kybella, the recently FDA-approved injectable that dissolves neck fat, has been running a television commercial that stars the impressive before-and-after shots of a man looking to make his genetic double chin history.

Meanwhile, at cosmetic surgery community RealSelf.com, interest in nonsurgical hair-loss treatments have skyrocketed among male readers, who are most concerned with taking action against their receding hairlines. Consumers visited the site to research and view before-and-after photos twice as much this year as compared with last year, show site metrics.

Follicular Unit Extraction — or FUE — remains the gold standard for both hair transplantation and hair restoration, says Austin, Texas, plastic surgeon Jennifer L. Walden, MD. SmartGraft is a new device that more quickly harvests hair grafts and places them in balding areas, so hair follicles have a better chance of surviving for fuller results. Either procedure starts at about $8,000 to $10,000 and can quickly rack up much higher, depending on the size of the treatment area and the time it takes to do the procedure.

Younger men aren’t leaving those hairlines to chance; Walden says she sees more men with premature thinning who want to intercept the process before it gets too visually noticeable. “Patients are more apt to call or consult for hair-loss treatments today and not feel embarrassed — cultural shifts have occurred, and cosmetic procedures are just more accepted and understood by the mainstream public,” says Walden.

Alex, a 30-something who works in financial services in New York City, recently had the popular FUE hair transplant. As someone who is sharply groomed and who works out regularly, he saw it as an extension of how he likes to present and take care of himself, both in his personal and professional life. “It’s vanity. I didn’t like the thinning and recession of my front hairline, so I handled it,” he tells Yahoo Beauty.

He paid $11,000 for the procedure and worked from home while recovering for a few days afterward, though he says the raw redness of his scalp took a couple of weeks to subside. And he learned that cosmetic surgery isn’t a magic wand and that expectations must be managed. “It is definitely an improvement, and I’m glad I did it — having said that, I expected there to be fuller coverage of the places where the hair follicles were plugged in,” he says. Would he consider other procedures? “If I felt Botox or a neck-lift were necessary at some future point, I’d consider — and maybe even more, if technology improves,” says Alex.

Cosmetic Surgery Times Declares Not All Men Are Created Equal

Contrary to the U.S. Declaration of Independence, not all men are created equal or identify themselves equally.

Dr. SteinbrechThese contradictions have propelled Douglas Steinbrech, M.D., FACS, a  plastic and reconstructive surgeon with locations in Manhattan and Beverly Hills, to create different categories for men to identify with on his website maleplasticsurgerynewyork.com & maleplasticsurgeryla.com.

“There are many different types of men,” Dr. Steinbrech tellsCosmetic Surgery Times. “Initially, the website was to educate men,” he says. “However, the website has also really increased the volume of men in my practice.”

Case in point, upon opening his practice, in Manhattan, 12 years ago, Dr. Steinbrech’s clientele comprised about 80% women and 20% men. Today, the practice treats roughly 80% men and 20% women.

Three years ago, to help drive male business, Dr. Steinbrech created four categories of males on his website: the model (or in Los Angeles, the actor), the athletic dad, the bodybuilder and the CEO/boardroom executive. Each category also lists several recommended procedures.

The Model

“The most popular procedure for the model is abdominal sculpting,” Dr. Steinbrech says. “Through hidden incisions, I sculpt the abs and achieve a dramatic reduction of fat to the flanks of the love handles.”

The second most popular procedure is a male model “makeover” that creates a chiseled Superhero jawline via fillers or permanent implants. Of the two, “we usually use fillers to inject into the cheekbone and accentuate the jawline and the chin,” Dr. Steinbrech explains. “I always tell my model patients that every guy has a better-looking ‘twin’ working in Hollywood. That counterpart has slightly higher cheekbones, a little wider jawline and a more projecting chin. By tweaking, we want to create this better-looking twin.”

Dr. Steinbrech says “twin” is the ideal word to convey to patients. “We don’t want the patient to look like someone else,” he explains. “We still want the patient to look like himself and appear natural; in other words, the best-looking version.”

The Althletic Dad

The athletic dad also wants sculpted abs, perhaps along with a stronger chest and some submental liposuction or Kybella (Allergan) to erase a double chin.

The “Daddy Do-Over” is Dr. Steinbrech’s version of the Mommy Makeover that includes liposuction and high-def liposcultping of the abs, love handles and neck. “These patients are now focusing on their jobs and families, so they have sort of let go the body they had in their 20s,” Dr. Steinbrech says.

The Bodybuilder

The third male category, the bodybuilder, entails intense sculpting. “I call my set of principals ‘BodyBanking,’ which is somewhat similar to bodybuilding but with fat,” Dr. Steinbrech notes. “With my patent-pending technique, I suck out the fat from the flanks, the lower chest and the abs, then I reinject that fat into the upper chest for a bigger chest, into the shoulders, the biceps, the triceps, the calves and even into the six-pack.”

According to Dr. Steinbrech, BodyBanking prevents the fat from returning to the areas where it was removed, adds bulk to the newly transferred-fat areas, and keeps fat from popping up into new areas like intra-abdominal visceral fat.

Silicone implants for the chest, calves, forearms, abdomen, shoulders, biceps and triceps are also popular in this category.

The Executive

The executive type of male is seeking a cleaner neckline and fresher eyes, which often means an upper and lower blepharoplasty and/or a neck lift. “This patient wants to look better in the boardroom and be able to compete with the younger guys,” Dr. Steinbrech says.

In June, the website added a fifth male category: “The Biggest Winner,” a play on the popular TV reality show “The Biggest Loser,” which targets men who have gone through massive weight loss, mostly as a result of gastric bypass, with lingering excess skin that is addressed with any or all of the various body part lifts.

“These patients require circumferential body lifts,” says Dr. Steinbrech, who has developed a swimsuit lift, which conceals incisions lower, beneath the swimsuit line. “Currently, a lot of physicians make the same incision then they would for a woman—high upon the thigh and wrap around as if the man would be wearing a women’s one-piece bathing suit.”

Of the five categories, the male model represents about 30% of Dr. Steinbrech’s patients, followed by the athletic dad (20%), “The Biggest Winner” (20%), the body builder (15%) and the executive (15%).

Dr. Steinbrech encourages other cosmetic practices to understand their own local male patient types, based on practice location. “For example, if you practice in South Carolina, there may not be a lot of actors or models,” he says.

Stratifying patients also helps to create an understanding of realistic aesthetic outcomes and allows people to identify with specific procedures.

Gynecomastia, BE GONE!

All summer long. Scorching hot days. At the beach. At the pool. Even on vacation. No matter where he went or what he was doing, my cousin refused to take his shirt off. Pretty strange for a guy, huh? Well, not as strange as you just might think…

Gynecomastia. That’s what my cousin suffered from, and while it definitely made him feel like an outcast, the truth was he was far from it. Also known as “man boobs,” Gynecomastia is the condition of enlarged male breast tissue, and believe it or not, it affects an estimated 40 to 60 percent of the male population — leaving most men feeling embarrassed, emotionally distressed, and with low self-esteem. Although my cousin felt alone every time he would hide his body under a baggy shirt, there’s unfortunately plenty of guys out there doing just the same, feeling ashamed and uncomfortable in their own skin — pretty much living within their own personal prison.

Understanding Gynecomastia:

Here’s the basics on what you need to know — Gynecomastia is excess localized chest fat, excess glandular tissue development, and/or even a combination of both. It can be present in either one or both of the breasts, but it doesn’t have to be there to stay forever.

In severe cases of gynecomastia, the weight of excess breast tissue may cause the breasts to sag and stretch the areola (the dark skin surrounding the nipple). In these cases the position and size of the areola can be surgically improved and excess skin may be reduced.

Gynecomastia is a condition of over-developed or enlarged breasts in men that can occur at any age. The condition can be the result of hormonal changes, heredity, obesity or the use of certain drugs.

Gynecomastia only takes approximately 1-2 hours. Dr. Steinbrech will make a small incision on each side of your chest from which he will remove the excess fat. If more extensive tissue removal is needed, he will make incisions in the peri-areola position for inconspicuous healing. Your chest will be tightened and contoured with careful attention to natural, masculine results.

…And the recovery:

Your male breast reduction surgery will yield immediate results. Men are often astounded at the significant change in their chest and their more confident outlook on life after the procedure.

No overnight stay is needed after your gynecomastia surgery, but you should arrange for a friend or family member to drive you home after the surgery and assist you.

The total recovery time for gynecomastia surgery is 4-6 weeks depending on how your body is healing. Just after the procedure you will wear a tight male vest for compression; this is easily hidden under normal clothing. If only liposuction was performed, you will back to work the next day. Hours on hours, days on days, weeks on weeks, and even years on years — that’s how long some people spend in the gym trying to achieve physical perfection. While determination and dedication are major keys to success, let’s be real: sometimes we can all still use a little extra help.

Personally, I’ve lost count of the amount of times that I wished I could simply just transfer fat from one part of my body to another, and chances are, I’m sure you have too. And while I always thought it was one very far-fetched quick fix, the truth is, it’s not so crazy after all.

Gordon Ramsey Transforms His Face

We get it, Ramsay doesn’t seem like the type who would succumb to trivial things such as looking good or changing his appearance to please the public eye. Ramsay expresses how he would read the vicious things written about him in the tabloids, describing him as “craggy-face, map of Wales, ugly, deflated rugby ball”, as the list goes on and on. The gossip suggests that he has gotten Botox, facial fillers, veneers, a hair transplant and a facelift. We aren’t sure just how accurate these accusations are, but judging by his current appearance, we don’t think he went that far.

Until of course, a weird case of a snowball effect had spun the media and sent us all through a whirlwind. Strangely enough, shortly after his procedure, Ramsay was spotted leaving a hair clinic with a cap covering his scalp. Then, shortly following his exit route from the Beverly Hills clinic, he was spotted looking extremely puffy and swollen. His response to all the rising gossip surrounding his appearance? He was out shooting in Costa Rica where a group of bandits poured gasoline all over him. Following this odd-sounding story, things just started to sound even weirder. Not too long after this mishap, Ramsay and his family were vacationing for holiday where they spent two weeks in Napa. When the family had gone horseback riding, the vulgar chef explains how he had a terrible allergic reaction to the horse on top of his recent scalp problem, he refers to the long stint of unfortunate events as a “hair nightmare”.

Botox Beats Migraines

shutterstock_334723313It started in my teenage years: debilitating migraines, preceded by auras and distorted figures that suddenly appeared in my field of view. Sometimes I suffered from tunnel vision and lost my eyesight for a few minutes. When it happened I would lock myself up in a bathroom, shut off all the lights and push a towel across the bottom of the doorway to make the room darker. I even wore sunglasses to make my world as opaque as possible.

Then minutes later the pain would arrive – this searing, monster pain usually right at the back of my forehead. Sometimes it lasted for only four hours, and other times for half a day.

This is what a migraine feels like to me: it isn’t a “headache”, as some websites describe it. Migraines make me talk funny because my brain can’t properly function from the blood clotting that occurs in my head. My hands and feet either get tingly or go numb. It’s torture.

I’d tried everything for relief, from buying readers to lessen eye strain to visiting scads of doctors for various prescription medications. Nothing worked.

After the number of my migraines escalated to two to three times per week, I thought about trying more radical treatments. I’d heard celebrities talk about how Botox helped them with migraines, but I blew it off as an excuse to use it for more superficial reasons.

“No, I don’t get Botox to look young in Hollywood. I get it for migraines,” I watched a wrinkle-free starlet say once on a home shopping network show as she patted her forehead. I just rolled my eyes.

This is what a migraine feels like to me: it isn’t a “headache”, as some websites describe it. Migraines make me talk funny because my brain can’t properly function from the blood clotting in my head. My hands and feet either get tingly or go numb. It’s torture.

Until one day I flipped through a magazine and noticed an article with the headline “Migraine Sufferers” in bold, glossy letters. Naturally it caught my attention. It said that the FDA had approved Botox to treat chronic migraines.

Discovering this article in the midst of my constant migraines felt like a sign. I immediately made an appointment with a local plastic surgeon. I’d already tried Botox once and didn’t experience any side effects, giving me no apprehension about using the injectable again.

I visited the plastic surgeon and received injections to my forehead, head, neck, back and shoulders, leaving me feeling a little stiff for a few days. He told me it could be at my second round of injections three months later that I’d really notice results.

After twelve weeks and two sets of injections with barely any side effects, my migraines disappeared for about six months. This felt like a monumental win. I went from having migraines every few days to nothing for a half a year.

Other sufferers report similar results. “I have found in my practice that Botox is highly effective for patients with chronic migraines,” says Dr. Douglas Steinbrech of Male Plastic Surgery in New York City.

Why does it work? “Botox reduces muscle motion and strength in key areas of the face and neck where certain types of migraines are generated,” says Dr. Manish H. Shah of Shah Aesthetic Surgery in Denver. “Muscles that don’t squeeze as hard won’t compress the sensory nerves as much, reducing the pain associated with migraines.”

For those interested in trying Botox for migraines, Dr. Steinbrech explains, “I use the Botox in areas of the forehead, temporal and back of the neck. Typically, the treatment takes about five minutes. The patient can get right back to work, and the relief can last up to four months.”

5 Quick Facts About Botox for Chronic Migraines

  1. Not all cosmetic doctors who do Botox are trained in the administration of the drug for chronic migraines. Make sure you properly describe your condition before scheduling your treatment.
  2. Each appointment shouldn’t take more than 15-20 minutes of your time.
  3. The Botox is only injected into small muscles and shouldn’t feel like more than a small prick. Side effects may include slight soreness, mainly in the neck area. Expect to receive the injections in several parts of your body.
  4. It takes two treatments over 12 weeks to get the full results. Set up a schedule to continue to get Botox treatments every 12 weeks to keep migraines at bay.

De-botch-ery at the Beach: Avoid Being Called Out

Boobs. They used to be a simple thing — something I have, something all other females have, something I never paid that much attention to, until shows like E!’s “Botched” came out.

If you’ve never watched the show, consider yourself lucky, because once you see some of the stuff on there, it can never be unseen — and I mean ever. While I’ve heard plenty of horror stories about plastic surgeries gone wrong, I never knew how real the problem was, until I saw it with my own eyes on the big screen.

Still, even though these botched boob jobs are without-a-doubt downright alarming to look at, I have to admit — I can’t get enough of ’em. For me, “Botched” is like that horror movie you love to hate; you know that it’s going to give you nightmares, but still, you just can’t look away. Even worse, now, I find myself looking everywhere I go.

You think I’m joking? While some may enjoy birdwatching, boob-watching is more the name of my game these days. And, this summer especially, I’ve been taking it much more seriously than I’d like to admit. Let’s just say that my beach days are no longer filled with simply taking a swim, but more along the lines of staring at all of the tata’s around me. Yes, I know how creepy this sounds.

The thing is, these popular shows like “Botched” and all of the celebrity plastic surgery makeovers that have been busting in the headlines have officially consumed my head; and now more than ever, I’m noticing what’s real, what’s fake, and what’s been completely butchered.

And let me tell ya, bad boob jobs are all around us, and I’ve been catching glimpses of them left and right… and as you can probably guess, it isn’t pretty.

Still, my issue isn’t with plastic surgery. If getting a little work done will give you that extra boost of confidence, by all means — go for it. The thing is, people really need to think twice about who it is they’re going to.

Steinbrech: “You take an average, good-looking kid, and you enhance his jaw, elongate his chin, enhance his cheekbones, and suddenly he looks like the cover of Vogue.”

  • The Body Builder: He wants similar procedures as the male model, but wants to look more muscular. Steinbrech says he also often removes the fatty tissue around the breasts from steroids.
  • The CEO or Board Member: These men are at the top of their career and they feel young and confident, but they’re worried they don’t look it. Steinbrech gives them the “Forbes Facelift”: a necklift, eyelift, jawline re-contouring, and perhaps liposuction as well.
 Steinbrech: “I tell these men I want to leave a little bit of wrinkles — since wrinkles are sexy and trustworthy.”
  • The Athletic Dad: These men are married with kids and have probably put on 15 pounds. He’s in his 40s and he’s thinking, “Man, I used to look so good in college.” Steinbrech says these men typically go for jawline re-contouring, liposuction, and a small eyelift.

Liposuction removes undesirable fat from the abdomen, waist, hips, buttocks, thighs, knees, upper arms, chin, cheeks and neck. By definition, the original feature defines the term dad bod as, “a nice balance between a beer gut and working out. The dad bod says, ‘I go to the gym occasionally, but I also drink heavily on the weekends and enjoy eating eight slices of pizza at a time.’ It’s not an overweight guy, but it isn’t one with washboard abs, either.”

The terms “dad bod” and “mom bod” may just be a fad and the topic of the moment, but the implications are worth holding on to.
